Welcome to the Georgia Department of Human Services, Office of Human Resources. The Office of Human Resources is responsible for assisting new hires and transferring employees as they complete the pre-employment process. The Office of Human Resources has implemented an online pre-employment process. Therefore, new hires and transferring employees have the ability to electronically complete and submit their pre-employment forms.

How to Complete Pre-employment Forms

All forms must be completed and submitted prior to your scheduled New Hire Orientation date.

  • Option 1: Email your completed forms to the HR Generalist identified in your Offer Letter. Note your forms must be emailed to your HR Generalist at least three (3) business days prior to your scheduled New Hire Orientation date.
    • Complete the Electronic Signature Agreement Form
    • Open and complete all fillable forms (transferring employees complete applicable forms only)
    • Save completed forms to your computer using the following file name format: (form name, your name, and date - Employee Information Sheet Jane Doe 01022017)
    • Email your completed forms to your Human Resource Generalist
      • Based upon the increase in file size of completed forms, you may need to send multiple emails to your Human Resource Generalist as the forms may exceed the size limit
    • Contact your Human Resource Generalist to confirm receipt of your forms

Although you have the ability to complete and submit your forms electronically, an additional option is provided below.

  • Option 2: Print and bring all your completed forms with you to New Hire Orientation.

Additional Forms Requiring Original Signatures
Please note an electronic signature will not be accepted for the following forms and thus should not be emailed to your Human Resource Generalist: 1) I-9 Form, 2) W-4 Federal Withholding Allowance Certification, and 3) G-4 Georgia Withholding Allowance Certification.  Additionally, completion of the I-9 Form requires identification verification. Therefore, please review the list of acceptable documents and bring the I-9 form along with the acceptable document(s) of your choice with you to New Hire Orientation.
